Patients have access to documents via the Patient Portal. Patient’s login using a User Id and Password provided by the provider. The patient's Home Page has a To Do list. Clicking on it will provide a list of links of documents to be completed. Documents are either reviewed, in which case they do not require a patient's signature, or electronically signed by the patient. (Some documents are co-signed by staff of the organization).
Documents include for Privacy, Consent for Treatment, Release of Information, Coverage, Financial Policy, etc. Another convenient document is the new patient information form. When cosigned, it uploads to the patient account page.
Patients may also fill out specific clinical questionnaires before the first visit.
